Menifee Mountain Memories Festival Vendor Application 

June 5^th & 6^th 

Hosted by the Frenchburg-Menifee County Chamber of Commerce 

Please note : Applications MUST be submitted to the F/M Chamber by: May 23^rd at 4:00pm. 

• Set up time starts at 12:00 PM on Thursday, June 4^th. 

• Food Vendors must be set up by Friday June 5^th at 12:00 PM for health inspection. They must comply with all Statewide Mobile Regulations. All food vendors, and selected vendors at the discretion of the F/M Chamber, will need to provide a certificate of insurance showing general liability insurance including products and completed operations coverage with limits of $1,000,000 per occurrence and $2,000,000 per aggregate. This certificate of insurance must also show the F/M Chamber as an additional insured under the current general liability insurance policy. 

• All vendors/exhibitors must supply their own tables, chairs, tents, etc. All tents/ canopies must be tied down securely. 

• Volunteer staff will be on-site during event, and security will be provided for Thursday and Friday nights. 

• There will be NO refunds. 

• No flea market or garage sale items. 

• No generators. Electric is offered at an additional charge. Vendor is responsible to match their plug to outlet. 

*ALL vendors are required to check in at the F/M Chamber office on day of setup. ALL vendors are to park in designated areas. Booths should remain open from 12 PM – 9 PM on Friday and 9 AM – 9 PM on Saturday.  

I, the undersigned, agree that I have read and understand the guidelines with this entry form, and I hereby agree to abide by them. I agree that neither the Frenchburg/Menifee County Chamber of Commerce, the City of Frenchburg, the Menifee County Fiscal Court, nor any employee or volunteer assumes any responsibility for any personal injury claims for injury or damage. I indemnify and take full responsibility of any damages from myself, any occupants with me, or any of my equipment, that may occur during the Menifee Mountain Memories event. Submissions of this form and registration payment are an indication of acceptance to the liability waiver above.
